After I have reviewed this questionnaire, i found there are few suggestion I would like to share and develop the questionnaire.
First question was asking the name of respondent which is the answer suppose to keep it secret. Name is often not required unless necessary.
Q 2: the question is related to illegal concern which answer may lead to respondents record and they may not providing fact.
Q 3: One of the common rule of questionnaire is DON’T ask open-ended questions unless necessary. Question that is require a lot of explanation often may not be answered because wasting the time.
Q 4: I would prefer Y/N question but DON’T ask leading questions.
Q 4.1: I prefer using tick form with this question as time is important.
Q 4.2: Rating scale is seem easy to answer if the question clear and concise. The shorter is the better.

Yes, your observations here are good. Another problem with Question 3 is that it is very difficult to answer. Perhaps we can't avoid asking difficult questions sometimes, but to ask it so early in the questionnaire is likely to deter some respondents from completing it.
